Thank you for
using Globaltech products. You have indicated that you have a Globaltech product
that has a fault and you are required to send it in for service.
Please refer to
the user’s guide for warranty conditions and note that the Globaltech
warranty applies only to Globaltech products and not to other equipment or
services that may be connected to it. Any faults with installation, wiring,
telephone lines, or any other products or services are not the
responsibility of Globaltech and may lie with the respective product or
service providers as the case may be. You are reminded that our warranty
does not extend to product misuse, neglect, water damage, accident
(including lightning strikes), improper application and or improper
installation.
If you believe that your product is covered by
warranty then please send a copy of your original proof of purchase (your
receipt) along with your product when you send it ti us for service or
repair.
IMPORTANT NOTICE!
By filling in
and submitting this form you are confirming that you understand and accept
the Agreement Herein. If you do not understand or do
not accept this agreement then do not fill and submit this form.

Please do not send your product
to us before you submit this Customer Service Request ("CSR") form, as our
service department cannot process any service work without a CSR.
After your product has been delivered to us,
it will be matched with your CSR, and then it will be serviced and or repaired promptly. Under normal
circumstances we will always try our best to complete any service within 10
working days. If your product is ready sooner or delayed longer you will be
informed accordingly. You must make full
payment for all charges
incurred before the product can be dispatched to you or picked up by you or
your agent.
Cost, Payment
& Service Terms:
I
hereby authorise Globaltech to repair the product(s) detailed below as long as total
Service charges of labor, parts and freight do not exceed $180 plus GST
per product. In the event that the charges subsequently assessed are higher than $180
plus GST, I must be informed and my consent must be given before any
work can go ahead. In the event that I do not wish to give my consent for
higher charges, I agree to pay the Assessment fee of $60 plus GST per product. All
charges must me paid in full prior to product release or return. In the
event that payment is not made within 60 days of product Service or
Assessment I agree that Globaltech may dispose of the product(s) in any way
they feel fit in order to recover unpaid charges.
All serial numbers of product submitted for repair must be entered below. In
the event that any product is covered by Globaltech warranty, all charges will be waved except for
$16 plus GST freight per
product as specified in the Globaltech Warranty.
